There are five levels, based on ''Anything but Tangerines'', ''Inflated Head'', ''Villi People'', ''The Flyin' King'' and ''Udderly Abducted''. The ''Puppy Love'' sub-levels are also included in this port. These levels are ported similarly from the original. This port has no ammo monitor, leaving the player no choice but to get hit. Although the player can use the Snot Swing is present as well.

The graphics and music are taken from the original game and simplified. However, there are several graphical glitches and you will notice how the music sounds nothing like the official thing except ''Puppy Love''. Like many other Super Game's products, this game has no ending; after the player defeats the seventh level, there's no ending whatsoever and the game simply resets. Earthworm Jim 2 (蚯蚓吉姆2) is a unlicensed port of the Sega Mega Drive game of the same name, made by Super Game for the Famicom and released sometime in 1997 by Realtec. It is the second Earthworm Jim 2 port, the first being produced by Ka Sheng, but unlike that version, it lacks the whip.
